Healthcare and human services

Care runs on context. Keep it close.

The people you support shouldn't have to carry their own story. We connect referrals, intake, care and operational systems so context travels with the person.

Who we help

Different services. The same commitment to people.

Your systems need to reflect the way you deliver support. A referral, a therapy session and a funded service each ask something different of your team.

NDIS providers

Your team balances the support a participant needs with funding, service agreements and the evidence behind each service. We help connect that operational work so staff can find the relevant information without rebuilding it across forms and spreadsheets.

Allied health

Your clinicians' day is built around sessions. Re-entering details, preparing follow-up and chasing referral information eat into the time between them. We help shape the work around those sessions, while keeping clinical records in the systems that own them.

Care and community organisations

Several teams, services and external providers may support one person. We help make responsibilities, relevant context and next actions visible, so continuity depends less on who happens to know the story.

“Your CRM in a clinical space is basically your lifeline.”

Where we can help

More of your day for the people you support.

From the first referral to the reporting behind your services, we can help with the work that keeps care moving.

Capture what intake needs, identify missing information and assign the follow-up. Your team can see which referrals are ready for review and which need attention, without checking every inbox.

See it in practice

A referral should move forward. Its context should come with it.

Health Cloud holds the client and the care plan, while the practice system stays the clinical record. They connect through MuleSoft, so Salesforce carries engagement, referrals and service delivery, not clinical notes.

Access with a purpose

A connected story doesn't mean open access.

Right context. Right person.

Intake, clinicians and coordinators have different responsibilities. The person's consent, each role and the purpose of access shape what appears.

In the example above, the coordinator receives operational context. Clinical notes remain outside that view.

How we work with you

Built around the way your service actually runs.

We start with the people doing the work: intake, practitioners, coordinators and the teams behind them. Together, we follow a real referral or service interaction and find where context, time or ownership gets lost.

That gives us the basis for deciding what belongs in Salesforce, what should remain in clinical, rostering or finance systems, and what needs to move between them. We work through access, consent and review responsibilities as part of that design. Changes can then be introduced around a defined workflow, supported with clear ownership and refined as your team uses them.

Technology that fits

Designed around care, and the work around it.

Start with your services, responsibilities and existing systems. Then choose where Salesforce and integration can make the work easier.

Sales and Service Cloud can support operational workflows. Health Cloud is an option when care-team and health-context requirements call for it.
